📋 Description

• Provide personalized care to geriatric patients in their homes or residential facilities • Work closely with the Director of Nursing to develop individualized care plans • Support each patient's unique goals and needs • Provide coverage for Home Health and Hospice patients in Kingman, Arizona • Complete visits typically lasting 45–60 minutes • Deliver care during 9-week Home Health episodes of care • Provide additional compassionate Hospice services • Travel throughout the service area and assigned territory • Participate in a weekend on-call rotation once per month

🎯 Requirements

• Experienced Registered Nurse (RN) • Active Arizona RN License • Valid Driver's License • Current Auto Insurance • Arizona Level One Fingerprint Clearance Card • Current CPR or BLS Certification • Current TB Test (skin test within 1 year or chest x-ray within 5 years) • Willingness to travel throughout the service area • Ability to provide coverage in the Peoria Buckeye area • Availability to take one weekend on-call rotation per month • IV experience preferred • Passion for high-quality, patient-centered care
