• Develop and maintain relationships with community host sites, tribal health programs, county health departments, libraries, senior centers, and other local partners • Coordinate event logistics with host organizations, including site accessibility checks and day-of communication • Manage outreach materials and channels, including flyers, event calendars, social media, local media, and partner newsletters • Identify local healthcare providers and specialty care facilities • Support participants in accessing referrals through warm handoffs, consent, and follow-through • Schedule and confirm participant telehealth appointments • Manage provider availability and telehealth coverage calendars • Administer the telehealth platform, including device setup, readiness and connectivity checks, and real-time technical support • Maintain backup workflows in low-coverage areas • Track outreach activity, event attendance, and referrals from scheduling through completion, including telehealth encounters, in BEM's reporting system • Participate in monthly coordination calls with AASCW and internal team meetings • Travel to mobile clinic sites throughout the four-county region
• 1–3 years of experience as a Community Health Worker or in a related community outreach, social services, health navigation, telehealth, or healthcare scheduling role • Relationship-building experience with diverse rural, tribal, and underserved communities • Comfortable troubleshooting technology, including tablets, hotspots, and video platforms, in field conditions • Valid driver's license and current auto insurance • Reliable personal vehicle • Able to travel at least 60% of the time across a multi-county rural region • Overnight hotel stays may be required depending on event schedules • Effective communication in meetings, by phone, and in person • Exceptional organization and time management • Attention to detail and accuracy at high volume • Self-directed, flexible, and effective with minimal supervision • Technological proficiency, including MS Office 365, scheduling software, and spreadsheets for data tracking • Positive interpersonal skills with a wide range of individuals • Results-oriented under firm deadlines, working independently and as part of a team • Must pass a background check • Bachelor's degree or higher a plus • Bilingual (Spanish or other) a plus • Familiarity with HIPAA and patient privacy practices a plus
