📋 Description

• Conduct comprehensive telehealth evaluations including history, physical examination, and diagnostic review • Develop and manage individualized care plans for patients with chronic lung conditions • Supervise respiratory therapists providing direct patient care • Collaborate with physical therapists and referring providers to ensure coordinated, comprehensive care • Provide patient education and counseling on pulmonary health, self-management, and lifestyle modification • Maintain accurate and timely documentation using BreatheSuite's EMR and Coaching Dashboard

🎯 Requirements

• Current certification as a Nurse Practitioner with active license in CO, AZ, or UT • Active advanced practice nursing license in good standing • DEA license preferred • 2 years of experience as a licensed NP • Strong clinical skills with experience in primary care, pulmonology, or chronic disease management • Experience supervising or overseeing other healthcare providers • Comfortable with or able to learn telehealth platforms, EMR systems, and remote monitoring tools • Access to a computer with webcam and microphone, and reliable internet connection • Minimum availability of 8 hours per week; 16+ hours preferred with consistent weekly availability.
