📋 Description

• Work comprehensively with providers to deliver value-based care management initiatives • Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care • Identify and coordinate community resources with patients • Provide patient education and health literacy on chronic condition management • Perform medication management, identifying potential medication concerns • Assist in ensuring timely delivery of services; Home Health, DME, Home Infusion • Resolve patients' questions and manage referrals and appointment scheduling

🎯 Requirements

• At least 3 years of direct patient-facing work experience • Active Multi-State/Compact License (LPN) (NCL) (LVN) • Technical aptitude – Microsoft Office Suite • Excellent written and verbal communication skills • Epic Experience (Plusses) • Bilingual (Plusses) • Additional single state licensures (LPN) (Plusses)
