📋 Description

• Receive referrals and engage clients within 24 hours through home visits or telephone contact • Complete documentation for contacts and case information • Conduct ongoing outreach and engagement through calls, home visits, and communication with the treatment team • Collect and review collateral information, including social histories, psychological evaluations, judicial records, treatment information, and DPS reports • Provide ongoing support and case management to individuals experiencing substance use • Develop individualized case management plans and coordinate substance abuse assessments • Develop and implement service plans, community referrals, reassessments, and follow-up • Coordinate client services, advocacy, and community resource information • Document services, write progress notes and summaries, and complete monthly progress reports and billing • Facilitate Intensive Outpatient and Outpatient Groups using evidence-based curriculum • Advise and support individuals and groups with substance abuse issues • Complete ASAM, ASI, CIWA, CINA, and ACES assessment tools within deadlines • Coordinate, facilitate, and participate in Adult Recovery Team meetings • Promote quality improvement and perform other assigned duties

🎯 Requirements

• Minimum high school diploma or equivalent • At least 3 years of related experience working with multi-problem families in substance abuse, child welfare, or behavioral health • Associate’s degree in a human service or behavioral health-related area preferred • Knowledge of human behavior, social work theories and techniques, and community resources • Counseling, interviewing, assessment, and therapeutic relationship skills • Flexibility in working with each client’s uniqueness • Sensitivity to cultural differences • Fluency in Microsoft Office applications, including Word and Outlook • Ability to stoop, bend, squat, sit on the floor, and walk over uneven surfaces, stairs, and steps • Ability to visually and auditorily assess and interact with clients for safety and wellbeing • Willingness to work outside the office, drive substantially, work occasional evenings and weekends, and be on-call for emergencies • Ability to lift up to 25 pounds • Valid Arizona driver’s license, reliable transportation, and proof of registration and insurance • Level One Fingerprint clearance or ability to obtain fingerprint clearance, and ability to maintain clearance • Must be 21 years of age or older to drive on behalf of Catholic Charities
