📋 Description

• Support care management activities and assigned member teams • Conduct member outreach by telephone and home visits • Discuss care plans/service plans, next steps, resources, questions, and concerns with members • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries related to care plans/service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document member care needs • Document and maintain member records and distribute records to providers as needed • Follow contractual requirements, regulatory guidelines, standards of practice, and organizational policies • Provide education on benefits and available resources • Perform other assigned duties

🎯 Requirements

• Prefer candidates residing in California • Ability to work PST hours • Healthcare experience • Customer service experience • High School diploma or GED required • 1–2 years of related experience required • Ability to perform member outreach by phone and home visits • Ability to document and maintain member records in accordance with state and regulatory requirements • Ability to follow contractual, regulatory, and organizational standards and policies • Ability to identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Qualified applicants with arrest or conviction records will be considered in accordance with the LA County Ordinance and the California Fair Chance Act
