• Support care management activities and assigned member care teams to ensure provider services, continuity of care, and member satisfaction • Conduct member outreach by telephone and home visits • Document care/service plans, activities, member care needs, and records • Engage members regarding care plans, next steps, resources, questions, concerns, and ongoing education • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Respond to member and provider inquiries, requests, and concerns related to care plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings • Make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education about available benefits and resources • Perform other duties as assigned and comply with policies and standards
• High School diploma or GED • 1–2 years of related experience • For Arkansas Total Care plan: Bachelor's degree in social science/health-related field, or high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients • Must reside in Arkansas or a border city • Must reside in or be able to support members throughout Washington and Benton counties in Arkansas • Strong experience working with the Developmental Disabilities population preferred • Driver's license required • Child and adult maltreatment check required before hire and recurring • Drug screen required at hire and recurring • Ability to travel 30%
