📋 Description

• Support care management activities and assigned member teams to ensure provider and partner services are delivered • Perform member outreach by phone or home visits • Document care and service plans and related activities • Discuss care plans, next steps, resources, questions, concerns, and ongoing education with members • Coordinate care activities with healthcare providers, community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns related to care plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document member care needs • Maintain member records and distribute them to providers as needed • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Comply with policies, standards, contractual requirements, and regulatory guidelines • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED • 1–2 years of related experience • Familiarity with the Delaware market, including local healthcare providers, community resources, and members served in the area • Ability to work Monday through Friday, 8:00 a.m. to 5:00 p.m. ET, with a flexible one-hour lunch • Ability to work alternate shifts, including 10:00 a.m. to 7:00 p.m. ET, based on monthly coverage needs • Ability to perform member outreach telephonically or through home visits • Ability to document and maintain member records in accordance with state and regulatory requirements • Ability to follow contractual requirements and regulatory guidelines and standards
