📋 Description

• Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs • Evaluate member needs through phone or in-home visits and recommend or facilitate care plans • Conduct telephonic, digital, home, and other site-visit outreach • Develop ongoing care plans for high-acuity members • Identify providers, specialists, and community resources for mental health and substance use disorders • Coordinate among members, families, caregivers, community resources, and care provider teams • Monitor care plans, treatment side effects, complications, clinical symptoms, and outcomes • Facilitate care coordination and timely access to needed services • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on care and quality improvements • Comply with policies and standards and perform other assigned duties

🎯 Requirements

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements, with behavioral health experience required • For NJ Health Plan: Master's degree in Behavioral Health or Social Work and Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) required • Active New Jersey license required • Must reside in Camden or Gloucester County, or in a nearby bordering county • Availability Monday through Friday, 9:00 AM to 5:00 PM
