📋 Description

• Develop, assess, and coordinate care management activities based on member needs • Develop or contribute to personalized care plans/service plans • Educate members and families/caregivers on services, benefits, procedures, care options, referrals, and provider instructions • Evaluate member needs, barriers to care, available resources, and recommend or facilitate appropriate outcomes • Identify providers, specialists, and community resources needed for care • Provide psychosocial and resource support related to employment, education, housing, food, participant direction, independent living, justice, and foster care • Coordinate members/families/caregivers with care provider teams to ensure timely access to services • Monitor progress toward care plan goals and member status or changes in condition • Collaborate with healthcare providers on care plan revisions and refer members for further care management evaluation • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform on-site visits when appropriate to assess needs and collaborate with providers or resources • Perform other duties as assigned and comply with company policies and standards

🎯 Requirements

• Bachelor’s degree and 2–4 years of related experience • Graduate from an Accredited School of Nursing if holding clinical licensure • Equivalent experience acquired through applicable knowledge, duties, scope and skill reflective of the position level may qualify • Current state’s clinical license preferred • Ability to work Monday–Friday, 8am–5pm • Previous foster care experience strongly preferred • Applicants must work remotely from their home anywhere within the state of Missouri
