📋 Description

• Support community connection activities by connecting members to community resources throughout their care management journey • Provide members with known community resources and support the care team in identifying community support • Coordinate community outreach resources and promote awareness of care/services • Support members with community and care resource inquiries • Prepare and support member correspondence and educational materials • Document and maintain community resources in accordance with health plan requirements • Assist clinical nurses and social workers through outreach, community education, informal guidance, and member support • Conduct non-clinical general health assessments and refer members to appropriate care/services • Conduct non-medical assessments covering home safety, community/environment resources, transportation, employment, and related needs • Conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families • Visit individual homes and/or community organizations as needed • Perform other duties as assigned

🎯 Requirements

• Must reside in the state of Kansas • Travel within Shawnee, Osage, Douglas, Anderson, Franklin, Butler, Chase, Lyon, Greenwood, and Coffey counties • High School diploma or GED • Requires 1 – 2 years of related experience • Working knowledge of Social Determinants of Health (SDOH) barriers • Must comply with all policies and standards
