📋 Description

• Connect members to community resources supporting their care management journey and necessary care resources • Provide members with known community resources and support the care team in identifying community support • Coordinate community outreach resources and promote awareness of available care/services • Support members with community and care resource inquiries • Prepare and support member-related correspondence and educational materials • Document and maintain community resources in accordance with health plan requirements • Assist nurses and social workers with outreach, community education, informal guidance, and member support • Conduct non-clinical general health assessments and refer members to appropriate care/services • Conduct non-medical assessments covering home safety, community/environment resources, transportation, employment, and related needs • Conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families • Make visits to individual homes and/or community organizations • Perform other assigned duties and comply with policies and standards

🎯 Requirements

• Must reside in the state of Kansas • Ability to travel within Butler, Chase, Lyon, and/or Greenwood County • Travel up to 75% • High School diploma or GED • Requires 1–2 years of related experience • Working knowledge of Social Determinants of Health (SDOH) barriers • Ability to conduct non-clinical general health assessments • Ability to conduct non-medical assessments • Ability to perform telephonic and in-person outreach • Compliance with all policies and standards
