📋 Description

• Connect members to community and care resources in a cost-effective manner • Coordinate community outreach resources and promote awareness of care/services • Support members with community and care resource inquiries • Prepare member correspondence and educational materials • Document and maintain community resources according to health plan requirements • Provide assistance to nurses and social workers through outreach, community education, informal guidance, and member support • Conduct non-clinical general health assessments and refer members to appropriate care/services • Conduct non-medical assessments covering home safety, community/environment resources, transportation, employment, and other needs • Conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families • Visit individual homes and/or community organizations as needed • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED • 1–2 years of related experience • Working knowledge of Social Determinants of Health (SDOH) barriers • Ability to work remotely from home anywhere within the state of Missouri • Ability to provide telephonic support to foster care members • Availability Monday–Friday, 8am–5pm • Ability to conduct telephonic and/or in-person outreach • Ability to make visits to individual homes and/or community organizations as needed • Compliance with all policies and standards
