📋 Description

• Connect members to community and care resources cost-effectively • Coordinate community outreach resources and promote awareness of care/services • Support members with community and care resource inquiries • Prepare and support member correspondence and educational materials • Document and maintain community resources according to health plan requirements • Assist nurses and social workers with outreach, community education, informal guidance, and member support • Conduct non-clinical general health assessments and refer members to appropriate care/services • Conduct non-medical assessments covering home safety, community/environment resources, transportation, employment, and other needs • Conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families • Visit individual homes and/or community organizations as needed • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED • Requires 1–2 years of related experience • Working knowledge of Social Determinants of Health (SDOH) barriers • Ability to work Monday–Friday • Ability to work remotely from home anywhere within the state of Missouri • Ability to support foster care children from birth to 12 years old who have physical health needs • Comply with all policies and standards
