📋 Description

• Develop, assess, and coordinate holistic care management activities • Develop or assist with personalized service care plans for long-term care members • Educate members and families/caregivers about services, benefits, procedures, healthcare instructions, referrals, and service options • Evaluate member needs and available resources and recommend or facilitate care plans • Identify providers, specialists, and community resources for long-term care • Coordinate between members/families and care provider teams to ensure services are accessible • Provide resource support related to employment, housing, participant direction, independent living, justice, and foster care • Monitor care plans, member status, and outcomes and recommend updates • Interact with long-term care healthcare providers and partners • Collect, document, and maintain member information and care management activities for regulatory compliance • May perform on-site visits to assess member needs and collaborate with providers or resources • Provide leadership feedback on opportunities to improve quality of care and cost-effective service delivery • Perform other duties as assigned

🎯 Requirements

• Bachelor's degree and 1 year of related experience, or equivalent experience acquired through applicable knowledge, duties, scope and skill • For Iowa Plan: Bachelor's degree and 1+ years of experience with populations served, or RN with 6+ years of experience with population served • For Physically Disabled/Elderly: RN licensed in Illinois; or bachelor's/master's degree in a human services-related field; or LPN with one year of experience conducting comprehensive assessments and providing formal service for the elderly • For Brain Injury/HIV/AIDS: RN licensed in Illinois with a bachelor's degree in nursing, social work, social sciences or counseling, or four years of case management experience; or certified/licensed social worker with a bachelor's degree or master's degree; or unlicensed social worker with a bachelor's degree • Experience working with addictive and dysfunctional family systems, racial and ethnic minorities, homosexuals and bisexuals, persons with AIDS, and substance abusers • Must comply with all policies and standards • Must meet applicable state, federal, and third-party payer regulatory requirements
