📋 Description

• Provide skilled nursing care to patients in their homes under the direction of an RN • Implement individualized care plans with patients, families, and the healthcare team • Educate patients and families on disease management, treatment options, and self-care techniques • Maintain accurate patient-care records, including visits, interventions, and outcomes • Communicate promptly with patients, physicians, clinical managers, and healthcare-team members about needs and status changes • Participate in care coordination and discharge planning • Report to the Clinical Manager • Support patients' recovery, rehabilitation, and independence in a home-health setting

🎯 Requirements

• Current nursing license in the practicing state • Valid driver's license • Auto insurance • Reliable transportation • Current CPR certification • Two years of experience as an LPN/LVN in a clinical setting • Must maintain personal vehicle liability insurance meeting state minimum limits or specified Humana limits, whichever is higher • Must undergo tuberculosis screening if selected for the patient-facing role
