📋 Description

• Plan and administer prescribed physical therapy treatment for patients in their homes • Assess and document patients' physical strengths to support physician evaluation and guide treatment goals • Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient-team goals • Provide PTAs and Home Health Aides with written care instructions • Oversee PTA and Home Health Aide performance and documentation • Document patient observations, interventions, and evaluations promptly and thoroughly • Report patient progress to physicians, the Clinical Manager, care team, patients, and families • Submit evaluations, plans, progress notes, and discharge summaries • Help patients regain strength, mobility, and independence through personalized home healthcare

🎯 Requirements

• Current and unrestricted Physical Therapy license • Current CPR certification • Experience collaborating with a team of healthcare professionals • Valid state driver's license • Maintain personal vehicle liability insurance in accordance with residing state minimum required limits or specified higher limits • One year of experience as a Physical Therapist (preferred) • TB screening required if selected • Ability to work 40 scheduled hours per week
