📋 Description

• Oversee medication therapy, strategy on comprehensive medication reviews, and prescription drug optimization for patients taking multiple medications • Promote health awareness with patients through Rx Education and targeted quarterly campaigns • Place outbound calls to confirm patients are taking drugs and provide counseling • Integrate programs developed to improve overall health outcomes with a focus on prescription drugs • Make decisions regarding own work approach and priorities while following direction • Work Monday through Friday with shifts assigned between 8:00 AM and 8:00 PM Eastern Time

🎯 Requirements

• Active, unrestricted LPN license • One (1) or more years' experience working as an LPN • Experience working with Electronic Health Record (EHR) systems for accurate documentation, data management, and workflow optimization • Demonstrated ability to solve complex problems and encourage collaborative problem-solving across and within teams • Proven ability to work independently while contributing effectively to team success • Mail Order Pharmacy experience (preferred) • One (1) year or more years of experience working as an LPN in a specialty pharmacy setting (preferred) • LPN Compact license (preferred) • Minimum internet speed of 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
