📋 Description

• Provide patient care through virtual visits • Conduct Medicare Wellness visits and acute same-day visits • Evaluate and treat center patients according to standards of care • Monitor medical care and quality using available data and chart reviews • Participate in and lead daily care-team huddles • Support the Associate Medical Director, Physician, and Center Administrator in maintaining a cooperative practice environment • Complete medical record documentation promptly and work with a quality-based coder to optimize coding specificity • Follow policies and protocols defined by Clinical Leadership • Meet with the Associate Medical Director regarding quality of care, outcome data, policies, procedures, and records issues • Maintain compliance with licensing, certification, and accrediting bodies

🎯 Requirements

• Bachelor's degree • 8 or more years of technical experience • 2 or more years of project leadership experience • Active, unrestricted Nurse Practitioner/PA license in the appropriate state • Required TB screening for this patient-facing role • Passion for contributing to an organization focused on continuously improving consumer experiences • Preferred: Masters in Science of Nursing /PA • Minimum home internet speed of 25 Mbps download and 10 Mbps upload for home employees • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
