âą Conduct inbound and outbound calls to patients/members about medication therapy, side-effect management and adverse-event reporting âą Assess and evaluate member conditions, emphasizing medication therapy, through telephonic interactions âą Develop and implement comprehensive care plans âą Monitor patient progress through regular assessments âą Provide medication education using advanced telecommunication systems âą Review prescriptions for accuracy âą Check for drug-drug interactions âą Evaluate disease-state appropriateness, dosage, directions and therapy âą Use electronic resources and guidelines for professional decision-making âą Report to a Supervising, Specialty Operations Pharmacist âą Participate in rotating Friday closing and Saturday shift coverage âą Participate in holiday rotation and mandatory overtime when required by business needs
âą Pharmacy degree from an accredited college of pharmacy (PharmD) âą Active Pharmacist license from the state of Florida, Texas, Ohio or Arizona Board of Pharmacy without restrictions or disciplinary action âą Minimum three (3) years of experience as a pharmacist in a high-volume retail, hospital or mail order pharmacy practice environment âą Participate in Federal prescription programs âą Comprehensive knowledge of Microsoft Office applications, including Word, Excel, Outlook and OneNote âą Knowledge navigating multiple systems, platforms and troubleshooting and resolving general technical difficulties in a remote environment âą Ability to work alone and within a team setting to meet department goals âą Problem-solving skills and use of resources for professional decision-making âą Focused, accurate and highly productive âą Effective verbal and written communication âą Ability to meet remote work internet requirements: minimum 25 Mbps download and 10 Mbps upload âą Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
