📋 Description

• Conduct, plan, organize, and coordinate healthcare accreditation and/or certification surveys in community settings • Travel to home health or hospice agencies being surveyed for accreditation/certification • Perform site visits in accordance with CHAP Standards of Excellence • Maintain strict confidentiality unless otherwise required by law • Comply with policies and procedures governing the accreditation/certification process • Prepare accurate, complete, and well-written survey documentation • Complete site visit preparation activities • Participate in training through a 4–6 week travel-based preceptorship during the first two months

🎯 Requirements

• Bachelor's degree in Nursing or a related healthcare field required • Active Registered Nurse (RN) license required • Minimum of five (5) years of experience in a compliance or regulatory role within the home health or hospice industry required • Minimum of one (1) year of leadership experience required, including responsibility for supervising staff and/or direct reports • Strong working-level knowledge of the CMS Home Health or Hospice Conditions of Participation and related resources • Strong attention to detail • Ability to deliver accurate, complete, and well-written documentation • Ability to work autonomously • Critical-thinking skills • Adaptability to change • Availability for a minimum of 10 survey days per month, excluding required travel days • Availability for 4–6 consecutive travel weeks during the first two months of employment for training (preceptorship) • Home health or hospice nursing professional with a strong compliance background
