📋 Description

• Provide high quality, continuity of care for patients through a telemedicine-first approach • Conduct two-way video-audio appointments with patients • Work collaboratively with patient care advocates and other medical team members to ensure patient care and follow-up • Engage in a fee-for-service model with robust incentive programs for top-performing providers

🎯 Requirements

• Completed an accredited nursing or medical program with a national certification • Currently licensed in Vermont with the ability to obtain additional state licenses as needed • Valid Vermont DEA Registration in good standing with all schedules to prescribe medications, including S2 • At least 2 years post-residency practice experience is strongly preferred • Must be Board Certified in Family or Internal Medicine • Commitment to evidence-based medicine • Willingness to learn how to diagnose and treat a variety of adult/primary care conditions • A PC with Windows 11 Pro or a Mac newer than 2020 is required
