📋 Description

• Provide occupational therapy services through a telehealth platform • Complete evaluations, reassessments, and treatment documentation as appropriate • Develop and implement individualized treatment plans based on patient needs • Collaborate with facility staff and interdisciplinary care teams • Monitor patient progress and adjust interventions as needed • Maintain accurate documentation and comply with applicable clinical and regulatory requirements

🎯 Requirements

• Active Occupational Therapist license in Michigan (required) • Previous skilled nursing facility (SNF) experience required • Prior telehealth occupational therapy experience required • Strong clinical assessment, communication, and documentation skills • Ability to work independently while collaborating effectively with facility teams • Proficiency with electronic medical records and telehealth technology
