📋 Description

• Lead a team of Care Navigators processing referrals to secondary care outside Crossover Health, including specialists, imaging, community resources, and employer benefits • Provide training, support, and guidance to the Care Navigation team • Maintain specialist, community service, and employer benefit directories • Audit data integrity, patient feedback, and referral documentation • Troubleshoot or escalate issues for the Care Navigation team • Manage staffing schedules for efficiency and optimal coverage • Establish and maintain relationships with identified service providers • Train relevant stakeholders on the external referral process • Ensure day-to-day staffing and appropriate coverage • Assist the team in processing a determined amount of referrals • Serve as a role model to achieve decreased healthcare spend, increased quality, and excellent patient and provider experience • Perform other duties as assigned

🎯 Requirements

• Minimum 3 years experience working in the healthcare field • Minimum 1 management or supervisory experience • Bachelor’s degree or relative medical experience • Experience in office management or administrative role • Excellent communication and people skills • Ability to multitask and prioritize • Self-starter with strong problem-solving skills • Excellent organizational and time management skills • May require standing, walking and sitting for extended amounts of time • Manual and finger dexterity and hand-eye coordination • Requires corrected vision, hearing and speech within normal ranges • Must be able to effectively communicate with patients and team members
