📋 Description

• Serve as an operational link between IDOH and regional partners • Support organizations receiving Rural Health Transformation Program funding with grant and contract requirements • Monitor state and federal compliance, allowable expenditures, budgets, documentation, milestones, deliverables, and timelines • Maintain audit-ready grant documentation and support consistent use of approved templates, tools, reporting systems, and grants-management practices • Identify performance risks, compliance concerns, budget issues, and implementation barriers, escalating issues to IDOH when appropriate • Collaborate with statewide Technical Assistance Providers—IHA, IPHCA, and IRHA—to support regional coalitions • Participate in regional consortium meetings, workgroups, and planning sessions • Convene Regional Committee meetings • Coordinate regional data collection, performance tracking, outcome measurement, and reporting aligned with CMS Rural Health Transformation Program requirements • Help partners understand CMS documentation, metric, narrative, and reporting expectations • Support corrective action planning when progress falls behind expectations • Serve as day-to-day liaison among IDOH, regional coalitions, healthcare providers, community organizations, and other partners • Help partners interpret state guidance, solve implementation challenges, share effective practices, and use consistent statewide tools and evaluation approaches

🎯 Requirements

• Bachelor’s degree in public health, health administration, public policy, business administration, or a related field • 5 years of experience in grant management, contract oversight, program administration, or related work • 3 years of experience working with healthcare organizations, rural communities, or public health systems • 2 years of experience demonstrating a strong understanding of compliance, budgeting, and reporting requirements for state or federal grants • Ability to manage multiple projects simultaneously with strong attention to detail • Excellent communication, facilitation, and relationship-building skills • Willingness and ability to travel regularly within different regions of Indiana • Experience with CMS-funded programs or federal cooperative agreements preferred • Familiarity with Indiana’s rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations preferred • Experience collaborating with statewide associations or multi-stakeholder coalitions preferred • Knowledge of rural health delivery, care coordination, or health system transformation preferred
