📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry and diabetic eye exams • Help identify care gaps and connect members with appropriate care • Travel to members’ private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to cover Marquette and the counties of Alger, Baraga, and Marquette • Able to work safely and comfortably in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants • Able to travel locally and within licensed states as needed • MD or DO qualification implied by the required qualifications heading • Multi-state licenses preferred
