📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with members and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Build trusted relationships and help improve health outcomes • Visit health plan members in their homes and connect them to appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Nurse Practitioner or Physician Assistant qualification • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Flexibility to travel locally and within licensed states as needed
