📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Engage one-on-one with members, answer health questions, and build trusted relationships • Help identify care gaps and connect people to appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Candidates must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • MD or DO qualification is stated in the qualifications heading • Multi-state licenses preferred
