📋 Description

• Provide Part Time In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits involving one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Perform additional health-plan-ordered diagnostic tests, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Make referrals to members’ health plans and encourage loved-one participation when appropriate • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to be flexible to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Multi-state licenses preferred
