📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits involving one-on-one health discussions, medication and medical history reviews, and physical evaluations • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit people in their homes and connect them to appropriate care • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products, including spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to work safely and comfortably in members’ homes, including environments with household pets, secondhand smoke, and airborne allergens or irritants • Flexibility to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Multi-state licenses preferred
