📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with people one-on-one and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people in their homes to help bridge gaps in care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Build trusted relationships and improve health outcomes • Make referrals to health plans and encourage participation of family members when appropriate • Ensure primary care physicians are aware of patients’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to flexibly travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes containing household pets, secondhand smoke, and other potential airborne allergens or irritants • Multi-state licenses preferred
