📋 Description

• Provide In-Home Health Evaluations in members' homes • Engage with people one-on-one and answer health questions • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Join a national network of clinicians focused on improving health outcomes • Visit people nationwide and connect them with appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Conduct virtual visits as needed • Make referrals to health plans and encourage loved ones' participation • Ensure primary care physicians are aware of patients' health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • MD or DO qualification • Multi-state licenses preferred
