📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Answer health questions and help identify gaps in care • Build trusted relationships and connect members with appropriate care • Visit members in their homes across the local area and licensed states • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to be flexible to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to perform duties safely and comfortably in members’ homes that may contain household pets, secondhand smoke, airborne allergens, or irritants • Multi-state licenses preferred
