📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage patients one-on-one and answer health questions • Review medications and medical histories • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit health plan members in their homes to help identify care gaps and connect them with appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • MD or DO qualification implied by the Required & Preferred Qualifications section • Multi-state licenses preferred
