📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits involving one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Identify care gaps and make referrals to members’ health plans • Encourage participation of members’ loved ones and ensure primary care physicians are informed of health and quality-of-life findings

🎯 Requirements

• MD or DO qualification • Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Multi-state licenses preferred
