📋 Description

• Provide In-Home Health Evaluations in members' homes • Conduct approximately 45-minute visits with one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, and bone density screening • Conduct virtual visits as needed • Make referrals to members' health plans and encourage involvement of loved ones • Ensure primary care physicians are aware of members' health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to be flexible to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Multi-state licenses preferred
