📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits involving one-on-one health conversations • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Avoid prescribing medication, ordering lab tests, or altering current treatment regimens • Travel locally and within licensed states • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Help identify care gaps and make referrals to members’ health plans • Support communication with members’ primary care physicians and care teams

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Must be able to flexibly travel locally and within licensed states as needed • Must be able to conduct virtual visits as needed • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Multi-state licenses preferred • MD or DO qualification
