📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit members locally and within licensed states • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Connect members to appropriate care and help bridge gaps in care • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • MD or DO qualification • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and potential airborne allergens or irritants
