📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits in members’ homes • Build trusted relationships and engage one-on-one with members • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products, including spirometry, diabetic eye exams, and bone density screening • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Candidates must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • MD or DO qualification indicated by the required and preferred qualifications section • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Multi-state licenses preferred
