📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with individuals one-on-one and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit health plan members in their private residences • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes • Make referrals to the individual’s health plan and encourage participation of loved ones • Ensure primary care physicians are aware of patients’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Candidates must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Ability to travel locally and within licensed states, as needed • MD or DO qualifications • Multi-state licenses preferred
