📋 Description

• Provide In-Home Health Evaluations in members' homes • Engage one-on-one with members and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Make referrals to members' health plans and encourage participation of loved ones when appropriate • Ensure primary care physicians are aware of members' health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Physician qualification as an MD or DO • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to perform duties safely and comfortably in members' homes, including environments with household pets, secondhand smoke, and other airborne allergens or irritants • Multi-state licenses preferred
