📋 Description

• Provide in-home health evaluations for health plan members • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Provide Signify Health Diagnostic & Preventive Services products • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Help identify care gaps and connect members to appropriate care • Do not prescribe medication, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • MD or DO qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
