📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide and connect them with appropriate care • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed • Make referrals to health plans and encourage loved ones' participation in care • Ensure primary care physicians are aware of members' health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Multi-state licenses preferred
