📋 Description

• Provide in-home health evaluations in members’ private residences • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services • Help identify care gaps and connect members with appropriate care • Build trusted relationships and improve health outcomes

🎯 Requirements

• MD or DO qualification • Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide services including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Ability to safely and comfortably perform duties in homes that may contain pets, secondhand smoke, allergens, or irritants • Multi-state licenses preferred
