📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people one-on-one and answer health questions during approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Avoid prescribing medicine, ordering lab tests, or altering current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to be flexible to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Multi-state licenses (preferred)
