📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Must be an MD or DO • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Multi-state licenses preferred
