📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Build trusted relationships and help improve health outcomes • Connect members with appropriate care and identify gaps in care • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Flexibility to travel locally and within licensed states as needed • MD or DO qualification • Multi-state licenses preferred
