📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits in members' homes • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Avoid prescribing medicine, ordering lab tests, or altering current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Must be able to flexibly travel locally and within licensed states as needed • Must be able to conduct virtual visits as needed • Must be able to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • MD or DO qualification
