📋 Description

• Provide in-home health evaluations in members’ homes • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Engage one-on-one with members and answer health questions • Help bridge gaps in care and connect members with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Build trusted relationships and contribute to improved health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide services in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants • MD or DO qualification • Multi-state licenses preferred
