📋 Description

• Provide In-Home Health Evaluations in members’ homes • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits with one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Provide Signify Health services, including Diagnostic & Preventive Services products • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Help bridge gaps in care and connect people to appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • MD or DO qualification • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Flexibility to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed
